Electronic Document Agreement
VIA is required by law to provide you with certain written disclosures, documents and information pertaining to your employee health benefit plan, as well as the rest of the benefits offered. These documents include, but are not limited to information regarding the Patient Protection and Affordable Care Act (PPACA), HIPPA Privacy Notice, VIAcare’s Grandfather Status and Summary of Benefits and Coverage’s as well as Enrollment material and other plan information.
Paper Copy of Required Information Available Upon Request - No Charge. If you consent to receive the Required Information electronically, you can also request a paper copy of the Required Information by contacting the Employee Services/Benefits office at 362-2210. We will not charge you any fees for providing a paper copy of the Required Information. Your consent does not mean that VIA must provide the Required Information electronically - only that we may do so.
You may change your consent – No Charge. You may give or withdraw your consent to receive information electronically. The option to change your consent can be found under “My Profile” on the employee portal’s (MyVIA) main menu.
I consent to receiving electronic information from Employee Services and Benefits through this portal.
I would like to receive paper notification from Employee Services and Benefits. I understand that Open Enrollment and non-mandatory information will still be communicated through this portal.
